Pediatric Factors: The management of upper extremity problems is more
complex and outcome less predictable in the pediatric age group for a
variety of reasons. In the early management phase, lack of communication
increases diagnostic uncertainty, particularly in the staging of possible
nerve or tendon disorders. Lack of full cooperation may greatly affect the
final result. The risk of complication or failure is particularly increased
for procedures which require either regular exercise or a strict
immobilization protocol after surgery. Dressings and casts are much more
likely to become dislodged or soiled. Hypertrophic scars are much more
common in the pediatric population, and may seriously affect ultimate
outcome. Effects of surgery or injury on later growth may be progressive
and require treatment years after the initial problem appeared to have
stabilized.
