Shearing forces are always a factor in blunt trauma. Because they are tethered, extremity vascular branch points are particularly susceptible to shearing injury at a distance from the obvious trauma. This effect makes surgical revascularization prone to failure and magnifies the zone of poor local perfusion adjacent to the wound. |
