Godina reported a series of free flaps for extremity trauma, and found a close relationship between delay to flap and the incidence of complications, both infectious and microvascular. The high risk window appears to be the subacute phase, with open granulating wounds. Clearly, this also selected out polytrauma patients who were too unstable to tolerate early surgery.
